Being billed long after service rendered

I was hospitalized in 2002 received numerous bills after insurance payments, all bills were paid. Four years later I get a new bill never seen, never billed. Called the hospital and confirmed my insurance company. They billed the insurance company in 2006 for 2002 treatment and a now sending me a bill for co-payment. Is there a time limit on when an original bill can be sent for services provided four years ago?
