If my stepmother goes on Medicaid, can they take my father's house and assets if they have been separated for many years but are still legally married (NJ)?

My stepmom is a tenured teacher in Monmouth County, NJ who has suffered a stroke. She owns a home which was passed to her by her deceased mother. She left my father, but is still legally married to him. Her daughter (my stepsister) has power of attorney and is telling me that if Medicaid gets involved or if my stepmom lands in a nursing home that not only can Medicaid seize her property but they could take my father's home (the home in which I was raised by both my biological parents, which would rightfully be mine and my sisters' in the future.) What is the law regarding seizure of property by Medicaid?
