Is there a standard Affidavit form for submission of proof to INS that two people have known married couple since their conditional residence was granted based on marriage?


I am filing an Immigration and Naturalization Service Form I-751 to remove conditions of Residence. What type of Affidavit forms do I use to meet the requirements of submitting affidavits "sworn to or affirmed by at least two people who have known both of you since your conditional residence was granted based on marriage"? What is the form form for such an affidavit? Can I make one up?
